What recommendations for dietary intervention would you give Mrs. R? Would you use the Carbohydrate Counting method or would you follow the standard Exchange Lists for Meal Planning? Explain your answer.

(1.) Instead of weight loss as the primary goal, we should aim for near normal blood sugar levels and optimal lipid levels. (2.) In place of trying to reach an ideal weight, which may be unobtainable, settle for a reasonable weight with the optimal blood levels mentioned above. (3.) Individualize diet plans based on preferences, lifestyle, and medication and do not rely on preprinted diet plans. (4.) The carbohydrate counting method is popular and recommended because it allows more flexibility in that 15 g of sugar would be one carbohydrate and can be substituted for other carbohydrate foods, not just added to the diet. However, this method may be difficult to teach to Mrs. R. If she could comprehend the method, I would use it.

Answer to Question 2

A strict bland diet is infrequently followed today. The first rule: eliminate any food that causes a problem. Eliminate caffeine, alcohol, ASA, and cigarettes. Excessively highly seasoned foods should be eliminated if they cause a problem. Do not over eat at any one sitting. Smaller, frequent meals help some individuals but are not taught as a rigid requirement. Milk is OK if taken with a meal. Try to take her time and chew her food well. Eat under as relaxed conditions as possible. Do not eat anything before bed that will stimulate HCl production, as milk, meat or meat products, etc.
