Answer to Question 1

10/1/98 Admit Mrs. MG to a room in a medical unit.
I.V. D5W NS  100 cc/h
Phenergan 25 mg po q4h
Lomotil 5mg po qid for one day then 5mg bid
Bathroom privileges as able
NPO
RD to see patient

Answer to Question 2

IBW = 5' = 100 lbs + 3 x 5 = 115  10
IBW = Actual weight/IBW x 100 = 165/115  100 = 143.5
