If you had to counsel Mr. Y, what are some examples of behavior modifications you would use? Include in your discussion why Mr. Y would have to chew his food extremely well after the surgery.

Answer to Question 1

Eat in only one room in the house; do not eat in front of the TV; have someone needs to serve Mr. Y instead of him serving himself; do not eat family style; remove all food from the table except what is on the plate in front of him. In addition to the information in question 13, if Mr. Y did not chew his food well he could cause a blockage in the exit site of his pouch that may have to be opened with endoscopy.

Answer to Question 2

The same complications exist as with any surgery, such as infection, wound healing, etc. These complications are greater with the obese. The added complication of the patient eating too much and rupturing the staples is apparent. Not chewing food well enough and causing an obstruction is also a frequent problem, and having to throw up could cause a serious problem. In addition, if Mr. Y follows his gastric stapling diet as he should, the limited nutrient intake may slow down the healing process and cause complications, particularly if a subclinical deficiency existed prior to the surgery.
