Look up and record the RDAs for folic acid and Mg for a 73 YOM. Discuss how you could justify such a dose of folic acid and Mg. Mr. D is also receiving M.T.E.- 5 (see Appendix D).

Question 2

When hospitalized, Mr. D was given 50 mg of thiamin IM every day x 3d. What is the current RDA for thiamin for a 73 YOM? How could such a dose be justified, especially since he is to receive one amp of MVI-12 every day?

Answer to Question 1

RDAs for Folate = 400 g/d. Mg = 350 mg. Same argument as above except folate is important in cell reproduction and Mg with numerous enzyme systems.

Answer to Question 2

RDAs for Thiamin = 1.2 mg/d. The usual dose of thiamin given to a starved or alcoholic patient is 50 to 100 mg qd x 3d. Thiamin is essential for carbohydrate metabolism. Once a patient is hospitalized and is to be refed, it is very important that sufficient thiamin is present to utilize the additional food. Excess thiamin is flushed out through the kidneys and is not toxic at these doses. The increased fluids will also be causing the kidneys to excrete more of the water-soluble nutrients.
