Some parts of the form are completed by the patient, but others are completed by the registered dietitian and the results are scored and eva-luated by a registered dietitian. The data are subjective, but are vital in recognizing early signs of malnutrition and initiating early intervention. The form does not evaluate psychosocial status.

Oral intake does help maintain some normalcy for the patient, but this is not the main reason for using it.
