Answer to Question 1

C
When the entire colon and rectum have been removed, the patient would have an ileostomy. A colostomy is creation of an artificial anus on the abdominal wall. Removal of the colon and rec-tum causes fluid effluent, the opposite of constipation. Dumping syndrome is caused by a partial or total gastrectomy.

Answer to Question 2

1
Rationale: Clients with allergies to ragweed should be informed about a possible allergic reaction when taking echinacea to prevent colds and boost their immune system. Continued use should also be avoided in clients with autoimmune disease or clients taking immunosuppressant drugs, Dust, cat dander, and strawberries are common sources of allergens but do not cause cross-reactivity with echinacea.
