Mr. K is a 60-year-old diabetic client on the medical surgical floor of the local hospital. He was admitted for nausea, vomiting, and loss of appetite.
 
  He is suspected of being malnourished, and has lost more than 8 pounds recently since his symptoms began. The nurse practitioner has diagnosed him with gastroparesis and ordered a pureed diet with: 1. Liquid nutritional supplements three times a day. 2. High fiber to stimulate peristalsis. 3. High fat to provide calories for energy. 4. No spices to avoid gastrointestinal upset.

Chewing and swallowing difficulties in older adults increase the risk of food asphyxiation. Vo-miting is caused by infection or foodborne illness. Gastroesophageal reflux is caused by a wea-kening of the sphincter between the esophagus and the stomach. Hiatal hernia is caused by part of the stomach pushing up through the diaphragm; this is often related to gastroesophageal ref-lux.
