Answer to Question 1

2
Rationale: The goal of palliative nutrition is to aid in the relief of symptoms and promote comfort. Although massage and gentle stretching may help to provide comfort, rigorous exercise is impractical and may cause rather than relieve discomfort. Mouth care and lip lubrication helps to relive dryness of the oral cavity. Loss of appetite is not uncommon among clients with terminal illness. The need for rest should be at the discretion of the client.

Answer to Question 2

3
Rationale: Providing support for the client and their family is an important role for the palliative care nurse. Checking blood sugar, weights, or input and output are not the focus of palliative care.
