Rationale: When a client has conditions like diabetes mellitus or coronary artery disease, the dietician should be part of the team that plans for optimum client outcomes; that includes a nutritional assessment. The nurse needs to rephrase questions if a client does not understand what the nurse needs to know. Likewise, the nurse should seek appropriate sources to answer client questions and that may include a dietician. It would not necessitate a nutritional assessment. Client food preferences do not merit a nutritional assessment.
