Brian has benign prostatic hypertrophy. His prostate gland, which surrounds the urethra at the base of his bladder, has enlarged. By age 50, half of all men have some enlargement of the prostate gland; after 70, three-quarters do. As it expands, the prostate tends to pinch the urethra, decreasing urinary flow and creating a sense of urinary urgency, particularly at night.
Medical treatments for benign prostate hypertrophy include drugs that improve urine flow and reduce obstruction of the bladder outlet as well as medications that partially shrink the enlarged prostate by lowering the level of the major male hormone inside the prostate. In some cases, surgical treatment is necessary.
