Answer to Question 1

The problem of the premature release of information may have been an issue that
could have been resolved through better training. There may also have been a
need to limit access to the command center to better control the flow of
information. It may have been necessary to confiscate all cell phones. A rumor
control mechanism should have been in place. There should also have been more
formalized communications with the families and friends of the miners.

Answer to Question 2

Organizations in crisis have to speak with one voice. It is in that spirit that the
company had rules concerning the public release of information from the
command center. When the initial positive reports were received, people eager to
spread the good news used their cell phones to contact the outside world.
Unfortunately, the premature release of information did not give the company
enough time to verify the information. As it turned out, violation of established
procedures was harmful.
