Weight loss is not recommended during pregnancy. For women with gestational diabetes who are overweight or obese, a modest caloric reduction may improve glycemic control without increasing the risk of ketosis. Limiting the carbohydrate intake to 40 to 45 percent of total energy intake may improve blood glucose levels after meals. Carbohydrate is usually poorly tolerated in the morning so restricting carbohydrate (to about 30 grams) at breakfast maybe helpful. The remaining carbohydrate intake should be spaced throughout the day in several meals and snacks, including an evening snack to prevent ketosis during the night. Regular aerobic activity can help to improve glycemic control. Women who fail to achieve glycemic goals through diet and exercise alone may need to use insulin or an antidiabetic drug that is safe to use during pregnancy
