The surgeon notes Ms. Watson probably will not resume eating by mouth for at least 3-5 days. Using ASPEN guidelines, what would be your recommendation for nutrition support for Ms. Watson?

Answer to Question 1

 Elevated glucose and lactate levels due to metabolic stress; glycogenolysis/gluconeogenesis/lipolysis
 Elevated positive acute-phase proteins: CRP, fibrinogen
 Diminished negative acute-phase protein: albumin, prealbumin, protein
 Electrolyte abnormalities (due to fluid shifts/third spacing): potassium, phosphate
 Elevated WBC due to immune response

Answer to Question 2

 If Ms. Watson progresses easily to an oral diet she should not require the use of enteral nutrition support. Tolerance and adequacy will need to be monitored as she was admitted with an unintentional weight loss.
