The Apgar provides a quick assessment of the newborn's status by focusing on the body systems needed to sustain life: breathing, heartbeat, muscle tone, presence of reflexes, and skin tone. Each of the five vital signs receives a score of 0, 1, or 2, where 2 is the optimal score. A total score of 7 or more indicates that the baby is in good physical condition. A score of 4 to 6 means that the newborn needs special attention and care. A score of 3 or less signals a life-threatening situation requiring emergency medical care. The Apgar is most beneficial at when given at birth. For a comprehensive evaluation of the newborn's well-being, the NBAS is used. The NBAS includes 28 behavioral items along with 18 items that test reflexes. The baby's performance is used to evaluate the functioning of these four systems: autonomic, motor, state, and social. The NBAS can be used to determine harm from exposure to teratogens. It is most beneficial at a time after birth when the baby can feel comfortable and secure during testing.
