Primary polycythemia, also known as polycythemia vera, has unknown cause, with diffuse hyperplasia of the bone marrow resulting in the overproduction of both red and white cells with features similar to a neoplastic disease of the blood cells.
Secondary polycythemia is induced by any condition that reduces oxygen in the blood. This subsequently increases erythropoietin production, which eventually increases the number of red cells in the circulation. These may include emphysema pulmonary fibrosis, or any chronic lung disease; renal tumor; and congenital heart disease with mixing of the shunted blood.
