Iron exhibits a diurnal variation, with levels dropping during the day; therefore it should be drawn early in the morning. Iron absorbed from a meal can falsely elevate serum iron, so a fasting specimen should be drawn.

Chronic loss of blood leads to loss of red cells and their iron content. Iron is conserved and recycled very tightly in the body. To maintain normal stores it requires that only the normal amount be lost on a daily basis. Gradually iron stores will be depleted and iron deficiency will develop.
