Answer to Question 1

ANS: C
Poikilocytosis refers to a variation in the shape of red cells. Anisocytosis refers to variation in size; when this is present, the RDW should be increased and the patient is most likely anemic.

Answer to Question 2

ANS: C
Spherocytes can slightly elevate the MCHC because they have lost some membrane in the process of sphering and so the hemoglobin concentration tends to increase slightly. The MCV is usually not significantly affected (although microspherocytes exist). Spherocytes decrease the ESR and have no effect on the white count.

Though Candida and Aspergillus species are the most common fungal pathogens causing invasive fungal disease in the immunocompromised, infections due to previously uncommon hyaline and dematiaceous filamentous fungi are occurring more often today. Rare fungal infections, once accurately diagnosed, may require surgical debridement, immunotherapy, and newer antifungals used singly or in combination with older antifungals, on a case-by-case basis.
