Answer to Question 1

A national provider identifier (NPI) is a unique, 10-digit number assigned to healthcare providers by the CMS.As of May 2007, all healthcare providers must use NPIs on all patient billing forms. NPI numbers must be used by all covered entities. A covered entity is any provider who submits claims to Medicare. Specialists who receive referrals from other physicians must place not only their own NPI number on the CMS claim form but also the NPI number and name of the referring physician. Not only does each provider have an NPI, but each facility or medical group that bills for individual providers has a facility or group NPI. Both the rendering provider NPI and group NPI are entered on the CMS claim form in specific locations.
