Fundal height changes are reflective of fetal growth and correlate closely with the number of weeks of pregnancy from 20 to 24 weeks of gestation (Cunningham et al., 2010). Assessment of fundal height provides an estimation of gestational age in relationship to uterine size. Fundal height is measured by placing the end of a tape measure at the symphysis pubis and extending it to the fundus (Fig. 19.2). The uterus is usually palpable above the symphysis at 12 weeks of gestation, midway between the symphysis pubis and umbilicus at 16 weeks, and at the umbilicus at 20 weeks.

Laboratory studies during the initial visit are quite extensive. These screening tests are dependent on the disease prevalence within the screened population (Table 19.2). Laboratory testing throughout the pregnancy is dependent on presenting complaints and the developmental stage of the pregnancy. For example, a glucose screen should be done between 24 and 26 weeks of gestation, whereas an anemia screen (hemoglobin and hematocrit) should be done initially and again at 28 to 36 weeks. However, if the woman complains of a vaginal discharge later in the pregnancy, a gonorrhea and chlamydia culture could be done even if the initial screen was negative.
