Premenstrual dysphoric disorder (PMDD) is characterized by recurrent symptoms that occur during the luteal phase of the menstrual cycle and remit during menstruation. Even though many women express mood changes and other symptoms during the premenstrual phase, 5 to 9 fully meet the criteria for PMDD. Differential diagnosis includes bipolar disorder, thyroid dysfunction, premenstrual syndrome, exacerbation of unipolar depression, anxiety disorder, and cyclothymic disorder. Careful tracking of symptoms for at least 2 months is advised to determine a diagnosis of PMDD (Box 17.13).
