The optimum temperature for sperm production is approximately 2 to 3 degrees below body temperature. Sperm production is not directly related to underwear sizing. Sperm is not produced in the penis. There is truth in the patient's statement, so this is not an old wives' tale.

The testes produce sperm and testosterone. With one testis, there is a reduction in produced testosterone and sperm production. The patient will not be sterile if the remaining testicle is producing sperm. Estrogen replacement will not be indicated in the absence of a testicle.
