Isotonic saline is used because sodium excretion is accompanied by calcium excretion through the kidneys. If isotonic saline is not used, the patient is at risk for hyponatremia in addition to the hypercalcemia. Hypertonic solutions will not help facilitate the removal of calcium through the kidneys.

: This patient is at risk for developing hypocalcemia. This risk can be avoided if the patient is instructed to ingest milk and milk-based products, has adequate exposure to the sun, and avoids alcoholic beverages. The greatest dietary concern for this patient is the adequacy of calcium intake. Protein monitoring is not indicated.
