When planning care for the client experiencing a craniotomy, the nurse provides interventions that will help decrease IICP. Which intervention should the nurse use?
 
  1. Have the client sit in a chair three times a day.
  2. Elevate the head of the bed 30 degrees.
  3. Apply a cool cloth over the eyes.
  4. Prohibit visitation by the family.

The nurse is monitoring the client with IICP who is on mannitol infusions every 6 hours. The nurse plans to monitor the client for:

  2. cold hands and feet.
  3. jaundice and decreased bowel sounds.

2. The nurse would elevate the bed to 30 degrees to help prevent IICP.
3. A cool cloth to the eyes helps with eye swelling, not IICP.
4. Close family is encouraged to help keep the client calm and less anxious, but the family is taught to keep the atmosphere quiet to decrease stimulation.

1. Edema around the ankles and decreased pedal pulses are signs of poor circulation, not of dehydration.
2. Cold hands and feet do not reflect dehydration.
3. Jaundice is seen in liver failure, and the client might have decreased bowel sounds from inactivity.
4. Poor skin turgor and sunken eye sockets are symptoms of dehydration, which can be caused by mannitol. Cardiac arrhythmias are a sign of imbalanced electrolytes caused by mannitol.
