A client diagnosed with lung cancer is about to undergo a lobectomy. In order to decrease the client's anxiety, the nurse plans to:
 
  1. teach the client about the side effects of pain medications.
  2. teach the client how to cough and deep breathe.
  3. take the client on a tour of the ICU.
  4. tell the client the head of the bed will be elevated.

1. Teaching the client about side effects of medications could increase the client's anxiety, even though it is necessary.
2. Teaching the client how to cough and deep breathe is not an anxiety-reducing activity.
3. Taking the client on a tour of the ICU, introducing the client to the staff, and demonstrating the equipment used will reduce fear because the client will know what to expect.
4. Raising the head of the bed will not reduce anxiety.

1. The chest pain associated with pneumonia can cause the client to breathe shallowly, which can cause lung collapse, poor oxygenation, and an inability to move the secretions up and out of the lungs.
2. Pain medication might help the client sleep, but deep breathing and coughing exercises are needed to clear the lungs.
3. Pneumonia does not usually cause pain in the GI system. Anorexia is a general complaint of overall illness. Pain medication will not change that.
4. Pain medication does not affect fluid collection in the lung.
