The nurse is conducting a focused respiratory assessment on a 78-year-old client. When collecting the nursing history, the nurse should ask the client about:
 
  1. Past exposure to chemicals and smoking.
  2. Usual diet.
  3. Elimination habits.
  4. Liver infections.

Question 2

The client who has nervous system damage to the diaphragm asks the nurse why there is a feeling of not being able to take a breath. The nurse should explain that the diaphragm:
 
  1. Expands during expiration, causing the client to feel like there is not enough air entering the lungs.

1. The diaphragm rises during expiration to help the client expel air.
2. Malfunction of the diaphragm can decrease lung expansion, allowing for minimum of air on inspiration, which makes the client have feelings of not getting enough air.
3. The diaphragm does not have any effect on the trachea.
4. The diaphragm does not affect the expansion or collapse of the alveoli.
