The nurse is conducting a focused respiratory assessment on a 78-year-old client. When collecting the nursing history, the nurse should ask the client about:
 
  1. Past exposure to chemicals and smoking.

  3. Elimination habits.

The client who has nervous system damage to the diaphragm asks the nurse why there is a feeling of not being able to take a breath. The nurse should explain that the diaphragm:

4. The diaphragm does not affect the expansion or collapse of the alveoli.
