A client being prepared to have a bone marrow transplant expresses concern that the procedure will be painful. What should the nurse explain to the client?
 
  1. You will be given some analgesic, since it can be painful.
  2. The procedure is very quick and not very painful.
  3. It should not be any more painful than having an injection.
  4. A local anesthetic is given, and so you won't feel any pain.

During the change-of-shift report, the nurse learns that an assigned client has a hemoglobin level of 8 g/dL. Based on this information, the nurse should plan to:
 
  1. Keep the client on strict bedrest to restrict activity.
  2. Encourage ambulation to prevent thrombophlebitis.
  3. Space activities in order to conserve energy.
  4. Prevent exposure to infectious diseases.

2. Platelet abnormalities and inactivity place client at risk for thrombophlebitis.
3. Hemoglobin is the oxygen-carrying protein in red blood cells; 8 g/dL is well below the normal level of 12-15 g/dL, and would predispose the client to dyspnea, fatigue, and weakness.
4. Hemoglobin level does not measure exposure to infectious diseases.
