2. Vaccines against HIV infection are being investigated, but to date, none have been successfully developed. Education, counseling, and behavior modification are the key elements for HIV prevention.
3. Asking the client where she heard about a vaccine is confrontational and can destroy the trust the client might have in the nurse and health care system.
4. Asking the client why she needs the injection is confrontational and might be seen by the client as prejudicial.

1. Abdominal pain, nausea, and bloody diarrhea would be a sign of graft-versus-host disease (GVH).
2. Sleeping habits are not a sign of rejection.
3. Unless the client loses a great deal of weight, weight loss is not an early sign of GVH.
4. An early sign of GVH is a rash that begins on the soles of the feet and palms.
