Answer to Question 1

Answer: 350 mL

First subtract the intravenous fluids for medications from the daily total or 1500 mL - 100 mL = 1400 mL. Then divide this total in half to determine the volume of fluid permitted during the day shift or 1400 mL/2 = 700 mL. The remaining 700 mL needs to be divided over the evening and night shifts. Divide 700 mL/2 = 350 mL. This is the volume of fluid the client is permitted on the evening shift.

Answer to Question 2

Answer: 3

1. The abdomen is not a reliable area to assess skin turgor in an older client.
2. The upper extremities are not a reliable area to assess skin turgor in an older client.
3. Due to loss of skin elasticity with aging, the sternum provides the most reliable indication of skin turgor and hydration level.
4. The lower extremities are not a reliable area to assess skin turgor in an older client.
