2. The environment must be quiet when the nurse auscultates.
3. Cold hands would be uncomfortable when the nurse palpates.
4. Assessing the ear canals is inspection.

1. The client's eyes are checked without the nurse touching, so gloves are not worn unless there is obvious infected drainage.
2. When examining the hair and scalp, the nurse would wear gloves, as the client could have sores under the hair, or lice.
3. Unless the client has a respiratory infection, gloves are not necessary when auscultating the lungs.
4. The client's gait is checked by asking the client to walk around the room while the nurse observes. Gloves are not needed.
