2. The client will most likely act in the same manner later. The nurse should understand that the client's boundaries were violated.
3. Backing away is the classic sign that the nurse has gotten too close. The nurse should explain the procedure and wait for the client to signal that an approach by the nurse is accepted.
4. Sitting on the side of the bed will only increase the client's anxiety.

2. The physician orders the diet, not the individual meals.
