Answer to Question 1

Answer: C

Placing the baby on the maternal abdomen promotes attachment and bonding and gives the mother a chance to interact immediately with her baby. Removing the baby to the radiant warmer does not promote interaction. Allowing the mother a chance to rest immediately after delivery does not promote interaction. Taking the newborn to the nursery for the initial assessment does not promote interaction.

Answer to Question 2

Answer: C

The family is not receiving any information regarding the status of the client. The nurse should ask social services or the hospital chaplain to provide support while the family waits. The family should not be encouraged to go to the cafeteria or home. The client's status may not be stable, and therefore the family should not be escorted to see the client.
