Answer to Question 1

Answer: D

A client who is Spanish-speaking will require a medical translator to be available during the health history portion of the nursing assessment. While the nurse may need to increase the volume of speech due to client's age, this is not an appropriate action until it is determined that the client is hard of hearing. Educational material should be provided to the client in the native language. Asking a family member to translate during an assessment can violate the client's privacy.

Answer to Question 2

Answer: C, D

Nausea and vomiting is a common experience during the first trimester of pregnancy. The exact cause is unknown, but it is thought to be multifactorial. An elevated hCG level is believed to be a major factor, but changes in carbohydrate metabolism, fatigue, and
emotional factors also may play a role. Accupressure and ginger are two complementary therapies that the nurse can suggest to the client to relive the symptoms. There is no evidence that the client is experiencing hyperemesis gravidarum. The client should be taught to notify the healthcare provider if vomiting occurs more than once a day. Over-the-counter are safe to use during pregnancy to decrease the occurrence of nausea and vomiting.
