Answer to Question 1

Answer: A

The iris may change color with the medication; this is expected. It is not unusual for the eyes to change color; this is an expected reaction. The client has said the eye color is changing; it is condescending to ask if the client is sure. Asking the client what is meant by saying the eyes have changed color does not make sense; the client means the eyes have changed color.

Answer to Question 2

Answer: C

Applying antibiotic ointment and an eye shield would be indicated in the care of the client with a corneal abrasion. Flushing the eye with normal saline is indicated for a burn injury to the eye. Surgery is indicated for penetrating injuries to the eye. Bed rest with an eye shield would be indicated for the care of the client with a blunt trauma to the eye.
