Answer to Question 1

Answer: C

The client with shortness of breath will experience activity intolerance due to a lack of oxygen and fatigue. The nurse should consult with physical therapy for endurance and musculoskeletal function. Clients with respiratory disorders often need an increase, not a decrease, in calories to maintain body functions. The client will be weak, so the nurse should not encourage strenuous activity. The nurse would want the client to be as independent as possible and would not encourage dependence with activities of daily living. It will often be appropriate to cluster this client's care to allow for periods of rest.

Answer to Question 2

Answer: B, C, D, E

Males in the 35-50 age range are least likely to experience a health problem from an iron deficiency. Vegan diet plans can lead to deficiencies in certain nutrients, including iron. All vegetarians should ensure that they get adequate amounts of iron, and to facilitate the absorption, vitamin C should also be plentiful in the diet. Children can be iron-deficient and may need supplements. Iron plays a role in red cell formation, and deficiency is most often manifested as iron deficiency anemia. Older clients are prone to developing iron deficiencies because of socioeconomic, physical, and emotional factors. Adolescence is a time of rapid growth physically, emotionally and socially. Nutritional needs increase, especially for iron, due to blood loss during menses, the increased metabolic rate of growth, and poor nutrition intake.
