Answer to Question 1

Answer: D

During a seizure, the nurse remains with the child, watching for complications. Be sure nothing is placed in the child's mouth during a seizure. Suction equipment should already be set up at the bedside before a seizure begins. The child should not be restrained during a seizure.

Answer to Question 2

Answer: B

The client who is experiencing cholelithiasis should be instructed on the relationship between increased fat consumption and the severity of pain associated with cholelithiasis. While all clients should be instructed to reduce sodium intake, decreasing sodium will not assist in reducing cholelithiasis or its pain. Increasing fluids will not assist in reducing cholelithiasis or its pain. While all clients should cease smoking, it has no relationship to cholelithiasis.
