Answer to Question 1

Answer: C

The postpartum woman is at risk for over-distention, incomplete bladder emptying, and buildup of residual urine (urine that remains in the bladder after voiding). Glycosuria is expected for a client during pregnancy, not during the postpartum period. Nocturnal enuresis and risk for hyperkalemia are anticipate for older adult clients.

Answer to Question 2

Answer: D

Alternative therapy including music therapy has been used for nausea and vomiting. Ondansetron is a serotonin receptor agonist used commonly and an antiemetic. Clients should be positioned with the HOB elevated 30 degrees during enteral nutrition feedings. Enteral nutrition and TPN are used concurrently. Nausea is worsened by drinking fluid within 1 hour of meals.
