Answer to Question 1

Answer: A

A client who is fasting is at risk for development of metabolic acidosis. The body recognizes fasting as starvation and begins to metabolize its own proteins into ketones, which are metabolic acids. Starvation would not result in respiratory acidosis or alkalosis or in metabolic alkalosis.

Answer to Question 2

Correct Answer: 2

The first call made by the nurse should be to the primary care provider, who will decide whether the nurse should notify the family or to personally make the call. After the primary care provider is notified, the family should be called next. They will inform the nurse of what funeral home will be used. The coroner will be notified at a later date, and the nurse will not need to make the call in most instances.
