Answer to Question 1

Correct Answer: 3

The nurse applies pressure to the site only if bleeding occurs, and holds pressure until bleeding stops. Needles should not be recapped, because this increases the risk of accidental needlestick injuries. Massaging the area increases the risk of ecchymosis and speeds absorption, which is not desirable in this instance. The skin should be depressed, not pinched, when the needle is removed.

Answer to Question 2

Correct Answer: 3,4

The sites for intradermal injection are the forearm, upper chest, and the back beneath the scapulae. The forearms are not appropriate for this client, but the other two sites are acceptable. The thigh, calf, and abdomen would not be appropriate for intradermal injection sites.
