Answer to Question 1

Correct Answer: 4

The thigh would be best. Although the lower leg could be used, it is not as accurate as the thigh. The client with bilateral mastectomies should not have blood pressure measured in the arms due to the reduced lymphatic flow as the result of removal of the axillary lymph nodes.

Answer to Question 2

Correct Answer: 2

Because of the risk of spray from the inserted chest tube, the nurse should wear gloves, mask with eye shield or mask and goggles, and a gown. There is no need for the nurse to wear sterile gloves during the procedure.

There can actually be a 25-hour time difference between certain locations in the world. The International Date Line passes between the islands of Samoa and American Samoa. It is not a straight line, but "zig-zags" around various island chains. Therefore, Samoa and nearby islands have one date, while American Samoa and nearby islands are one day behind. Daylight saving time is used in some islands, but not in others—further shifting the hours out of sync with natural time.
