This exam should not cause tenderness, which is an indication of inflammation. The presence of extreme hardness or nodules is characteristic of prostate cancer. Enlargement of the prostate will cause urinary tract symptoms such as difficulty in starting a stream, or dribbling of urine. Urinary tract infections will cause painful and frequent urination.
