Ideally, to allow for a holistic approach to assessing and caring for the client, biographic data should include information related to sexual health, including sexual orientation label. However, discussion of sensitive information, including that related to sexual health and practices, requires the nurse to first establish trust and a rapport with the client.

While all of these questions are appropriate to include in a health history, the only question that is appropriate while conducting a review of systems is the item that asks about a history of respiratory issues.
