ICD-10-CM category M1a (chronic gout) requires the assignment of a fourth, fifth, sixth, and seventh character to complete the code. It is incorrect to report a category M1a code without the fourth through seventh characters. Thus, which code is assigned for idiopathic chronic gout of the right shoulder, without tophus.
