One of the reasons the ICD-10 has many more codes than the ICD-9 is:
 
  a. more codes are assigned.
  b. laterality.
  c. there are more illnesses.
  d. the rule-out codes.

Question 2

If a procedure was done but laterality is not documented by the physician:
 
  a. code unspecified.
  b. code separate codes, one for each side.
  c. assume it is the right side.
  d. assume it is the left side.
