vMatt is a 45-year-old male whose wife recently passed away after an extended battle with breast cancer. He was her primary caregiver at the end. He is at the office today because he is having difficulty sleeping. He reports he has lost weight over the past several months, and overall, just does not feel right. He says he feels really lost since his wife died. They were married for 25 years. He also says, She made me promise that I'd get help if I got depressed after she died. I don't like admitting that I'm having problems coping, but I promised her. With tears in his eyes, he also said, I'm keeping that promise, no matter what.
 
  Based on the circumstances, Matt was diagnosed with grief reaction with depressed mood. A prescription for antidepressants was given, and Matt was counseled on potential side effects. He is to return to the office in one month; he was also instructed to call sooner if he develops any medication side effects. He was also urged to get counseling and/or join a group of people who have suffered recent bereavements.
