Patient is a 33-year-old black male who presented to the ER with complaints of malaise and dyspnea. Physical exam showed swelling in the lower legs, and pulmonary edema. Patient has a history of rheumatic heart disease. He had rheumatic fever when he was 10. EKG showed ventricular tachycardia. Echocardiogram showed right ventricular enlargement. A diagnosis of acute rheumatic myocarditis was made. The patient is being admitted for treatment with diuretics, digoxin, and ACE inhibitors.
