Answer: Level II codes are national codes used to report medical services, durable medical equipment (DME), supplies, ambulance services, and other services NOT contained in the Current Procedural Terminology (CPT) code book.

Answer: Representatives are from the Blue Cross/Blue Shield Association, the Health Insurance Association of America (HIAA), and the Centers for Medicare and Medicaid Services (CMS).
