Following the First Report, the physician follows up with progress reports (sometimes called supplemental reports) every two weeks or each time the patient is seen by the doctor. Although many states have forms for progress reports, they may also allow a narrative report to be filed rather than requiring completion of the specified form. Progress reports are always sent at the end of a hospitalization even if the patient is expected to be readmitted later. This report often serves as both a report on the patient's condition and as a bill.
