While assessing a 14 year old with a known history of diabetes mellitus Type 2, the nurse notes darker skin around the teen's axilla. The nurse should:
 
  1. Contact the doctor immediately
   2. Provide a wash cloth and soap to cleanse the area.
   3. Document the findings and note that acanthosis nigricans is present.
   4. Ask the patient if he/she has had a recent infection.
