In a newborn nursery, a nurse indicates she hears a soft murmur in one of the newborns. After obtaining blood pressures on all four extremities, she finds that the blood pressure is higher in the right arm than the right leg.
 
  The nurse knows this can indicate: 1. Nothing, as this is a normal finding.
   2. Coarctation of the aorta.
   3. Ventricular septal defect.
   4. Shock due to poor perfusion in the lower extremities.

2. This is a predominant sign of COA due to a narrowing of the aortic arch after the innervation to the upper extremities, thus resulting in a lower blood pressure in the lower extremities.
3. This is not a sign of VSD.
4. Shock would indicate a low blood pressure in all extremities.
