The nurse has been caring for a child on the pediatric floor and notes that the child's PMI has shifted to the midline. The nurse is aware that this can indicate:
 
  1. Poor inspiratory effort by the child.
   2. Pneumonia in the left lung.
   3. Pneumothorax.
   4. Neurological integrity has been compromised.

1. Point of maximum impulse is the heart's sounds.
2. Pneumonia may mask the heart's sounds, but should not indicate a shift in the PMI.
3. Air is very heavy and will shift the heart to the midline of the sternum, resulting in a shift of the point of maximum impulse.
4. PMI does not refer to any neurological alterations.

NSAIDs should be given with meals to avoid gastric irritation.
